Patient name: Thomas Small
Medical history: The patient is a senior in high school and was diagnosed with
MD when he was 2 years old. He is now completely confined to a wheelchair
and sees his medical doctor every 3 months and a physical therapist who assists
Thomas in maintaining his muscle and joint strength and how to stay active. He
lives with his father. His mother passed away when he was 12 years old.
Thomas was close to his mother and still suffers from depression with the loss of
his mother. He does not see a psychologist due to financial strains. His father is
a blue-collar worker and struggles to pay his son’s expensive medical bills.
Thomas was recently diagnosed with cardiomyopathy (dilated type) which is
being treated with medication. Thomas wishes to go to the prom and graduate
high school this year.
Medications: metoprolol (Lopressor) – beta-blocker for cardiomyopathy.
Prednisone for muscle weakness and cardiac issues, OTC pain relief agents as
needed, Desyrel (Trazadone) for depression which his MD gave him because his
father cannot afford a psychologist
