Discuss how you would perform the risk identification, risk analysis, and risk
control steps of the risk management process addressing the causative errors
mentioned in the case.
The risk of mistaken patient identity in the clinical setting is a common error that has
negatively affected healthcare quality. Risk identification can be performed through conducting
iterative checking and confirmation of patient details, their diagnosed illness, their prescribed
medication or treatment procedure, their background, building consensus with the patient on the
required medical procedure, checking the patient chart, and checking the consent form (Levy &
Pravikoff, 2018). Performing these checks would definitely help in identifying the error or
mistaken patient identity.
Risk analysis needs to be done contextually based on the level of error that has already
been committed by the time risk identification is complete. Risk analysis should include
identification of the correct patient identity, organizational issues that have caused the risk, the
severity of the risk, the inconveniences caused by the risk, and the corrective course of action to
remedy the risk (Levy & Pravikoff, 2018).
Risks control would entail putting in place procedures that would help the patient
alleviate their pain, suffering, complications, and potential risks resulting from the mistaken
identity error. The risk control phase would also look into correcting the organizational,
individual, and machine related factors that could have led to the occurrence of the risk (Levy &
Pravikoff, 2018). There is also need to arrange for patient compensation as guided by the damage
caused by the risk.

Is it a one-time analysis or an ongoing process?
Risk identification, risk analysis, and risk control is a one-time analysis based on the
patient case in question. This is because different patients will be exposed to different types of
risks, which would require different approaches to manage (Levy & Pravikoff, 2018). However,
it is worth noting that risk management is an ongoing process that requires constant assessment
of organizational systems, processes, human resources, and equipment to identify potential
sources of risks and initiating corrective action to mitigate and prevent such risks.

What red flags in connection with the patient’s care should alert the healthcare
providers to take action to remedy the situation to patients?
Some of the red flags that should alert the healthcare providers is resistance of patients to
undergo medical procedure, lack of written information on patient’s chart, lack of patient consent
forms, patient is missing in the expected ward, mismatch in name spelling, lack of consistency in
patient information and opinion, missing consent forms and many others (Levy & Pravikoff,
2018). Such red flags should compel healthcare providers to double check the patient’s
information to ascertain that they are dealing with the right patient.
