Integrating SNAP
EHR System in
Public Schools
Project Management Team Members
Superintendent
● The Superintendent will be the executive leader of the implemented team.
● Will be responsible for coordinating all administrating actions needed for SNAP
Electronic Health Record System.
● Will provide leadership and guidance facilitating the system.
● Will work closely with Snap Representative and district superintendent’s to
implement program.
● Make sure we are HIPAA compliant.
● Present SNAP EHR system to faculty, students, and parents at the beginning of the
school year.
SNAP Representative
● Will educate and guide executive staff and district administrators.
● Will help to facilitate and guide leadership team with putting together
implementation plan.
● Will be the lead trainer for executive staff and administrators.
● Will assign lead SNAP EHR system trainers to each district with in NYC DOE.
● Present with Superintendent Snap EHR system to faculty, students, and
parents at the beginning of the school year.
Nurse
● Will work closely with Superintendent, executive staff, and Snap
Representative, and IT to implement SNAP EHR system into schools.
● Will have prior experience working and using SNAP.
● Help guide a facilitate the leadership team with the implementation
of SNAP EHR system.
● Will Facilitate SNAP EHR and introduce to School nurses and faculty.
● Will be the leader and contact point for all school nurses.
IT Leader
● Will be responsible for all technology that deals with SNAP EHR
system.
● Tasks will include: software integration, employee password
implementation and logins, and tech issues.
● Will hire and assign IT representatives for each district and
each school with in their districts.
Executive Teacher Representative

Lead teacher representative on executive staff.
Create a cabinet for all teacher leaders from each district.
Will assign a lead teacher representative for each district.
Will work closely with school nurse with the integration and
implementation of SNAP EHR system.
